Our office is happy to submit claims to your insurance company on your behalf. In order to provide you this service, it is necessary
for the billing staff to have complete and accurate insurance information. Our staff will ask to make a copy of your insurance
card when you arrive for your visit so please be sure to have this with you. If you have no insurance coverage, please contact our
billing staff before services are rendered to discuss a payment plan. Please note: Patients are expected to pay their copayment
at the time of service.
Our office participates with a number of local insurance carriers. Please contact our office to check our participation status
with your particular insurance plan.
